I understand that the information Dr. Jeni Weis provides is for educational purposes only, and is never a substitute for medical advice from a physician. 

I understand that Dr. Jeni Weis is a Traditional Naturopath (natural medicine doctor), not a physician or a medical doctor, and I am aware that Dr. Jeni Weis is not licensed to practice medicine. 

Definition and scope of practice of a Traditional naturopath: A Traditional naturopath is a doctor who educates their patients about natural health and nutrition to facilitate the body’s own health restoration process. Traditional Naturopaths do not perform surgery, prescribe drugs, or practice medicine in any form. It is illegal for a traditional naturopath to recommend a patient stop taking prescriptions. That decision is between a patient and their physician. Holistic modalities used by naturopaths can include herbal medicine, nutrition and lifestyle recommendations, reflexology, iridology, hydrotherapy, kinesiology, and non-invasive testing including hair mineral analysis. Traditional Naturopaths are not medical doctors or physicians, and do not treat, diagnose conditions or cure disease in any form. Traditional Naturopaths seek to find the root cause of illness and educate holistically on lifestyle factors and nutrition to support their patient’s health. 

I have read the above definition of a traditional naturopath, and am fully aware that Dr Jeni Weis does not treat, cure, diagnose, prescribe drugs, perform surgery, or order invasive testing. 

I am aware that Dr. Jeni Weis follows HIPAA privacy guidelines and will not share or sell any private patient information with any third parties without written consent.

I understand that there are no refunds available for services and education provided through Dr Jeni’s Inner Circle

I authorize reoccurring membership charges to automatically process on my card/account on file on the same day that I begin my membership with Dr Jeni's Inner Circle unless requested otherwise by emailing admin@holistichealthcircle.com or by texting 219.327.8935:

1 Month Subscription will auto process on the same day each month that membership begins.

3 Month Subscription will auto process every 90 days from the date membership begins.

1 year subscription will auto process every 365 days from the date membership begins.
